Bridges to Excellence (BTE)

Bridges to Excellence (BTE) is medical quality initiative by the Healthcare Incentives Improvement Institute (HCI3). BTE focuses on recognizing excellence in the care of chronic conditions. The BTE program operates with the understanding that high-value care for chronic conditions reduces the multifaceted costs of medical care and enhances overall health. BTE recognitions strive to measure and improve the most high-impact health indicators, while providing meaningful incentives to clinicians.

BTE recognizes excellence in the following areas:

  • Asthma
  • Cardiac care
  • Congestive heart failure
  • Cardiology Practice Recognition (Endowed by the American College of Cardiology)
  • COPD
  • Coronary artery disease
  • Depression
  • Diabetes
  • Hypertension
  • Physician Office Systems
  • Spine care
  • Medical Home Recognition

Each BTE program offers three performance thresholds, allowing participants to achieve Level I, II, or III recognition. Clinics that achieve Care Recognition as well as Office Systems Recognition are eligible to receive designation as a BTE Medical Home.
