Commonly serving as the patient's entry point into the health care system and as the ongoing point for healthcare service provision, primary care physicians are responsible for addressing a plethora of personal health care needs and developing relationships with patients and partners in the communities they serve. Primary care physicians specialize in comprehensive, continuing care for people of all ages and collaborate with other health professionals, utilizing consultation and referral services.

To exacerbate the situation, over the past decade most primary care providers have been forced to implement template based EHRs, driven by the federal government's push towards digitizing our nation's Electronic Health Records. Template based EHRs are not user friendly and they interfere with the patient-provider relationship and damage high quality patient care. In fact, a major study by the American Medical Association found that primary care providers spend twice as much time with their EHRs than they do with their patients, and the template EHRs utilized by physicians were described as clunky, rigid and cumbersome. Unequivocally, a physician's focus shouldn't be on EHR templates but rather on delivering high quality care to the patient at hand.
